Parent or LEGAL Guardian MUST be present with a valid government issued photo ID and the minor must have a birth certificate present.
If the guardian's name is not on the birth certificate, additional documents such as adoption papers or court orders may be required to prove guardianship. The minor must also provide a photo ID, such as school ID.
Acceptable forms of ID are: Photo ID, Driver's License, Passport, and Military ID.
No ID = no piercing.
